One of 4 types of identification is required to check out material from the UI Libraries. All must be currently valid.
University Issued Faculty, Staff, and Student Cards:

Questions about your borrower's status can be answered at the Main Library South Circulation Desk, Hardin Library, or any branch library during normal business hours. Main Library Circulation Services is located on the first floor of the Main Library.
We strongly recommend against loaning your ID card or your library materials to others. All financial responsibility for library materials rests with the person whose ID card was used to check them out.
If Your Student ID is Lost or Stolen:
If your UI Student ID Card is lost or stolen, please notify University ID Card Services, 335-2716. The cost of replacing a lost/stolen card is $20.00. If you currently use your ID card for security access, you should notify the contact person at the security access site anytime your card is lost, stolen or if you replace an ID card.
Prior to obtaining a replacement ID card for one that has been lost or stolen, you should call University ID Card Services to deactivate the lost/stolen ID. Once a UI Student ID Card has been reported lost or stolen, you must appear in person at University ID Card Services, 253 Iowa Memorial Union, to have a new card issued or to remove the hold on the deactivated card.
If you suspect theft and/or unauthorized usage of your UI Student ID Card, please contact the Department of Public Safety, 335-5022.

CRIMINAL CHARGES OR DISCIPLINARY ACTION
If a lost or stolen UI Student ID card is found, it should be sent or taken to University ID Card Services or the ID Card Administrator, 253 Iowa Memorial Union.
If Your Faculty/Staff ID is Lost or Stolen:
If your UI Faculty/Staff ID Card is lost or stolen (this includes cards for retirees and affiliates), please notify the ID Card Administrator, 335-0835. There is NO charge to replace a lost or stolen UI Faculty/Staff ID Card. If you currently use your ID card for security access, you should notify the contact person at the security access site anytime your card is lost, stolen or if you replace an ID card.
Prior to obtaining a replacement ID card for one that has been lost or stolen, you may call the ID Card Administrator to deactivate the lost/stolen ID. If you find your ID prior to replacement, you may call to reactivate the card.

If your UIHC Badge is lost or stolen, notify Hospital Human Resources, 356-2008 (after hours, call Facility Safety and Security, 356-2658). The first lost/stolen UIHC Badge will be issued at no charge. There is a $10.00 charge for replacing a second lost or stolen UIHC Badge within 12 months of the first replacement. If you currently use your ID card for security access, you should notify the contact person at the security access site anytime your card is lost, stolen or if you replace an ID card.
Prior to requesting a replacement for a lost or stolen UIHC ID Badge, you may call Hospital Human Resources to deactivate the ID. If you find your ID prior to replacement, you may call to reactivate the card.

Library Issued Cards:
Borrower's Permits: Users not affiliated with the University may, under some circumstances, be permitted to check materials out of the Libraries. Application for a borrower's permit may be made at Main Library Circulation Services or any university library. Borrower’s permits, in most circumstances, will be issued to:
Proxy Card: Faculty members may authorize graduate assistants or other staff members to check out materials for them. The faculty member or assistant may obtain an application for a proxy card from the Main Library Circulation Department or any university library. The faculty authorization form
is also available on-line. The faculty member must fill out the application and assumes financial responsibility for any materials charged in this manner.
Lost or Stolen Library Issued Cards: If your paper bar-coded UI Libraries card is lost, stolen, or mutilated, please report this as soon as possible to the issuing library or to Main Library Circulation Services. A new card can be issued and all activity blocked on your old card.
