{"id":744,"date":"2013-05-07T10:50:26","date_gmt":"2013-05-07T15:50:26","guid":{"rendered":"http:\/\/www.lib.uiowa.edu\/forms\/?page_id=744"},"modified":"2025-06-06T13:29:24","modified_gmt":"2025-06-06T18:29:24","slug":"proxy","status":"publish","type":"page","link":"https:\/\/www.lib.uiowa.edu\/forms\/proxy\/","title":{"rendered":"Faculty Authorization Form"},"content":{"rendered":"<p><strong>Proxy Card Information:<\/strong><\/p>\n<ul>\n<li>Proxy cards are valid at the Main Library, branch libraries, and the Law Library.<br \/>\nProxy cards for Law faculty are issued at the Law Library.<\/li>\n<li>If you later wish to have the card expire before the expiration date given below, please <a href=\"http:\/\/www.lib.uiowa.edu\/services\/circ\/\">contact circulation staff <\/a>who can cancel the card and prohibit any further check-outs or holds to be placed from the proxy card.<\/li>\n<li>All circulation notices will be sent to the faculty member, not to the proxy.<\/li>\n<li>A list of items checked out or requested via the Proxy card can be viewed by clicking <a href=\"http:\/\/infohawk.uiowa.edu\/myaccount\">My Account on InfoHawk<\/a>. To access the proxy account use the three sets of three numbers from the pink barcode number on the proxy card as the Hawk ID and password.
