Proxy Cards and Borrowers' PermitsA Proxy Card (Faculty Authorization Card) is issued to an individual at the request of a faculty member. This card will allow the designated user to check out library materials on the faculty person's library record for them. Cards may be issued at any Branch Library by select permanent library staff, as well as at the Main Library South Circulation Desk. Blank Faculty Authorization Form Borrowers Permits may be issued to any adult resident of the State of Iowa with proper documentation. Others with certain affiliations may also acquire borrowers permits. Patrons with borrowers permits may not check out as many library materials as faculty/staff/students, and their access to various software and databases may be restricted. Specifics vary according to policies/license agreements currently in effect.
